Here's how to submit your case to claim the bundle promotion
1. Log into or register for your CyberConnect account at provider.cyberdent.com.au and select 'Submit a case'. Enter the patients first name and surname, DOB and gender are optional.
2. Select the tooth on the odontogram for the crown, then select 'Full Contour' from the menu options on the right. Complete the prescription form for a Zirmil crown and select 'Add' at the bottom to add it to the case prescription,
3. Select the arch on the odontogram for the occlusal splint, then select 'Appliances' from the menu options on the right. Complete the prescription form for an occlusal splint and select 'Add' at the bottom to add it to the case prescription,
4. If there are any additional instructions or resources, you can add them to the case file.
5. Ensure you have both the crown and splint listed, then enter the discount code 'BUNDLE15'.
6. Let us know how you'll be sending your records, scans, TRIOS, or physical records. Follow the system prompts.
